What is Shockwave Therapy (ESWT)?

Shockwave therapy, also known as Extracorporeal Shock Wave Therapy (ESWT), is a non-invasive medical treatment that uses acoustic waves to stimulate healing and reduce pain in certain musculoskeletal conditions and soft tissue disorders. Shockwave therapy devices transmit rapid and intense sound waves directly into the affected tissues. These sound waves induce microtraumas, leading to an increase in blood flow and triggering an acute inflammatory response, all of which contribute to facilitating the healing process.

Shockwave therapy finds widespread application in various medical fields such as physiotherapy, podiatry, sports medicine, urology, and orthopaedics. It serves as a valuable method for delivering rapid pain relief and enhancing mobility for patients. Shockwave therapy, also known as extracorporeal shockwave therapy (ESWT), has been used to treat various medical conditions with promising therapeutic effects. The therapeutic effects of shockwave therapy include:

Substance P:  Shockwave therapy works by diminishing the concentration of the neuropeptide Substance P at the injury site, a primary cause of pain in the affected tissue. Additionally, the treatment triggers chemical changes that effectively reduce tissue inflammation, leading to even greater pain alleviation.

Neovascularisation: It refers to the formation of new blood vessels in the treated area. When shockwaves are applied to the affected tissues, they stimulate the release of angiogenic factors, which are substances that promote the growth of new blood vessels. 

Cox II Enzyme Regulation: Shockwave therapy has been found to modulate the expression of cyclooxygenase-2 (COX II) enzyme in tissues. COX II is involved in the production of inflammatory mediators called prostaglandins. By regulating COX II, shockwave therapy helps reduce inflammation in the treated area.

Overstimulation of Nerve Fibers: Shockwave therapy delivers intense mechanical energy to the tissues, which can overstimulate nerve fibers temporarily. This overstimulation disrupts the pain signals being sent to the brain, resulting in pain relief and decreased sensitivity in the treated region. (Gate control Theory)

On average, a single session of shockwave therapy in Canada may cost anywhere from $80 to $200 CAD. This all depends on geographic location, if there are other combination of therapy involved in the treatment, type of clinic, etc. Many clinics offer package deals for multiple sessions, which can bring down the individual session cost.

Radial Pressure Wave: 

The generation of radial pressure waves is generated by the collision of a projectile accelerated by compressed air or electromagnetic induction on an impact body (applicator). Most of the energy is transferred as a pressure wave into the adjacent medium. The wave propagates radially from the applicator, with the square of the distance the energy decreases.

Electro-hydraulic Shockwave: 

The oldest principle used in medicine is that in which the shockwaves are generated by a spark plug, the electro-hydraulic principle. The shock waves propagate in a medium (water) and are also focused in one place by a parabolic mirror.

Electromagnetic Shockwave: 

In electromagnetic shockwave generation the shock wave is emitted by a flat coil or a cylindrical coil by electrical pulses. In the flat coil a membrane is deflected which compresses a medium (water). The pulse is focused by means of an acoustic lens. In the cylindrical coil the shock wave is focused by a reflector.

Piezoelectric Shockwave: 

The piezoelectric principle is based on a focusing of pressure waves, which is produced by the electrical activation of piezoelectric crystals. The piezoelectric crystals are mounted in a shell in such a way that the pressure waves of each crystal meet in a focus.
